Chronic Care Management

Our office will soon be implementing a new service to our patients called chronic care management.  This is a service that will be available for Medicare and Blue Cross Blue Shield patients that suffer from two or more chronic medical conditions for 12 months or more.  Examples of a chronic medical condition would be COPD (chronic obstructive pulmonary disease), CVD (cardiovascular disease), diabetes, A-fib etc.

Your care management plan would be determined by your primary care provider, a nurse and the patient themselves to see what your care needs/support you the patient would need.

You will receive a personalized, comprehensive plan of care for all of your health issues.  Medications will be closely monitored also. 

How does this work?  A nurse will call you each month to check to see how you are doing and to see if you have any questions in regards to your health and medications.  You will be billed a minimal fee for this service.  Check with your insurance company to find out exactly what they will cover and how much you will be responsible for.  The purpose of the Chronic Care Management Service is to keep you healthy, decrease the amount of office visits to your primary care physician and to keep you out of the hospital.
